Example peak flow record

This is an example week of a peak flow record (note that a full peak flow record will normally be 4 weeks long). It can be given out to patients to make it easier to understand how to fill in the form. The first page of the document is a scanned in picture of the completed peak flow record with numbers indicating points of interest. The second page contains a description of the right and wrong things to do in common situations.
